As of June 15, 2013, the Pre-Existing Condition Insurance Plan (PCIP) allows you to see any provider of your choice for most covered services, as long as the provider agrees to accept PCIP payment. PCIP has prohibited providers from charging you an amount greater than the cost-sharing amount for covered services. Click here for tips on finding a PCIP provider.

*There is no benefit outside of PCIP's national DME network. If you choose a non-network provider to receive DME, you will be responsible for all charges.
^Transplant services must be provided by a facility participating in the LifeTrac Network with the "Select" program status. Benefits for transplants outside of this network are limited to $100,000. Kidney and cornea transplants are excepted from both the requirement to utilize a LifeTrac provider and also the $100,000 out-of-network maximum benefit.
